North Carolina Republican Leaders Push for Expanded Work Requirements in Medicaid

The proposed reform includes a mandate that adults without dependents work at least 20 hours per week or participate in job training or volunteer work to remain eligible for Medicaid benefits. The measure also includes provisions to offer job search assistance and workforce development programs to help individuals find employment. Supporters argue that these changes will not only reduce Medicaid dependency but also help improve recipients’ economic prospects by encouraging them to gain skills and secure stable jobs.

Senator Ralph Hise, a lead sponsor of the bill, argued that these new requirements are crucial for reducing welfare dependency and promoting personal responsibility. He stated that Medicaid should be a safety net for those truly in need, not a long-term support system for those capable of working. Hise further noted that several other states with similar work requirements have seen positive results, including lower Medicaid enrollment and higher employment rates.

However, the proposal has sparked intense opposition from Governor Roy Cooper and other Democratic leaders, who argue that the new work requirements would be burdensome for low-income individuals and families, particularly in rural areas with limited job opportunities. Governor Cooper has expressed concern that the new requirements could lead to many people losing their health coverage, particularly those who face barriers such as childcare responsibilities, transportation issues, or lack of available jobs.

Polling conducted by the North Carolina Health Access Coalition in January 2024 revealed a divided public opinion on the proposed reforms. While 54% of North Carolinians supported work requirements for Medicaid recipients, the same poll showed that 46% of respondents were concerned that such measures would disproportionately harm vulnerable populations. The survey also indicated that while there is support for increasing personal responsibility, there is wariness about the potential loss of healthcare coverage for thousands of working-class families.

As the debate over Medicaid work requirements continues, Republican lawmakers are pushing for a swift passage of the bill, which they argue will help ensure the long-term sustainability of Medicaid in North Carolina. Governor Cooper, on the other hand, is committed to blocking the measure, warning that it could push many North Carolinians off Medicaid at a time when access to healthcare is more critical than ever.
