One key figure in this complicated landscape is the pharmacy benefit manager (PBM). Initially designed to lower prescription costs and simplify access to medications, PBMs have evolved into influential entities that often prioritize profitability over patient care.

As the CEO of the Pontchartrain Cancer Center, I have witnessed firsthand the detrimental effects of PBM practices. They now exert significant control over how prescriptions are dispensed, the costs associated with them, and which medications are available to patients.

Despite my background and familiarity with the healthcare system, my daughter’s doctor and I found ourselves entangled in a prolonged battle with the PBM to secure approval for her treatment. After an arduous wait of ten weeks, her medication was finally approved in February, illustrating the delays many patients face.

The Systemic Challenges

The struggle with PBMs isn’t isolated; millions of Americans contend with similar obstacles. These delays can be life-threatening, particularly for those with critical conditions such as cancer or chronic diseases. The additional emotional toll on patients and families is immeasurable.

The problem originates from a system rife with redundancies, where responsibilities are shifted, creating a maze of bureaucracy that only hampers patients’ access to care.

Central to the ongoing issues with PBMs is their vertical integration. Major PBMs like Caremark Rx (CVS), Express Scripts, and OptumRx now not only manage drug benefits but also control the pharmacies that dispense medications and the insurers that pay for them. This consolidation of power introduces financial motivations that can detract from patient care:

  • Patients may be directed toward PBM-affiliated pharmacies, limiting their choices.
  • Access to alternative medications can be restricted as PBMs prioritize their products.
  • The overall cost of prescriptions continues to rise, exacerbating out-of-pocket expenses for patients.

The Financial Impact

A recent report from the Federal Trade Commission (FTC) unveiled how these dominant PBMs manipulate the drug supply chain, inflating costs for essential medications. In the period from 2017 to 2022, they generated over $7 billion in revenues through practices such as steering patients toward their pharmacies and employing spread pricing tactics. Despite bipartisan calls for reform, Congressional action has been lackluster, leaving patients in a precarious position.

A Call for Legislative Reform

There is a pressing need for Congress to address the challenges posed by PBMs. Recent hearings by the House Energy and Commerce Subcommittee on Health have illuminated the price inflation and barriers to care imposed by these entities. The reforms proposed hold significant bipartisan support and are aimed at fixing the existing issues that compromise patient care.

Looking Ahead

As healthcare continues to evolve, it is crucial to uphold the principle that patient welfare should be prioritized over corporate profit. Past missed opportunities to reform the PBM landscape highlight a systemic failure that must be rectified.
